What information I collect, use, and why
I collect the information in the ‘Your Details’ section of the Client Therapist Agreement to be able to provide a therapy service.
I collect audio recordings of our sessions
I record brief notes for my insurance purposes
Lawful bases
My lawful bases for collecting or using personal information for a therapy service is through the ‘Therapy client agreement contract’.
I cannot offer therapy services without holding data such as an email address or telephone number.
Where I get personal information from

How I store and how long I keep information
Any hard/paper copy documentation, contract or therapy notes is stored in a locked cabinet for a retention period of 7 years.
Digital data such as digitally signed contracts will be stored on an encrypted external hard drive which is also kept in a locked cabinet when not in use and retained for 7 years.
Session recordings are kept no longer than necessary.
Who I share information with
Data will not be shared with third parties unless required to do so by law. These instances pertain to The Terrorism Act 2000 ,Drug Trafficking Act 1994, Proceeds of Crime Act 2002 or Money Laundering Regulations 2007 , Childrens Act 1989 and amendments hereafter and the Mental Health Act 2005 and amendments hereafter, Safeguarding Vulnerable Groups Act 2006

Your data protection rights
Under data protection law, you have rights including:
Your right of access - You have the right to ask me for copies of your personal data.
Your right to rectification - You have the right to ask me to rectify personal data you think is inaccurate. You also have the right to ask me to complete information you think is incomplete.
Your right to erasure - You have the right to ask me to erase your personal data in certain circumstances.
Your right to restriction of processing - You have the right to ask me to restrict the processing of your personal data in certain circumstances.
Your right to object to processing - You have the right to object to the processing of your personal data in certain circumstances.
Your right to data portability - You have the right to ask that I transfer the personal data you gave me to another organisation, or to you, in certain circumstances.
Your right to withdraw consent – When I use consent as our lawful basis you have the right to withdraw your consent.
You don’t usually need to pay a fee to exercise your rights. If you make a request, I have one calendar month to respond to you.
